After five days of treatment with broad-spectrum
antibiotics, Abdullah's circulation was stable, but he
remained febrile. New cultures were taken. A few
hours later, his blood pressure decreased and
vasopressor treatment had to be increased.
After consultation with an infectious diseases
specialist, Abdullah's antibiotic treatment was
switched, and anidulafungin was initiated
empirically* (100 mg once daily**). Two days
after anidulafungin treatment was started,
investigations detected Candida albicans.
As a precaution, the abdominal
drain was withdrawn and the
central venous catheter
was changed.

Five days after antifungal treatment was initiated, clinical findings remained virtually unchanged.
Repeat blood cultures showed continued growth.
